Centers of Excellence Level of Coverage: 100% of contracted rates; $500.00 for family expenses for each day of in-patient medically necessary care (must be pre-approved by Executive Director). (Stanford Medical Center, Sequoia Hospital, O’Conner Hospital, University of California—San Francisco Medical Center and Children’s Hospital, and other pre-approved stays for specialized procedures and/or treatments.) 3.) Network Hospitals Level of Coverage: 80% of contracted rates. Salinas Valley Memorial Hospital is a Network Hospital effective April 1, 2011. 4.) Non-Network Hospitals Level of Coverage: 60% of UCR charges. 5.)
