Primary Network Hospitals Level of Coverage: 90% of contracted rates. (Columbia Good Samaritan Hospital {San Jose}, Columbia San Jose Medical Center, El Camino Hospital/Camino Healthcare Systems {Mountain View}, Lucile Salter Packard Children’s Hospital at Stanford {Palo Alto}, San Jose Health Care Systems, Santa Clara Valley Medical Center {San Jose}, and St. Louise Regional Medical Center {Gilroy}) 4.) Network Hospitals Level of Coverage: 80% of contracted rates. 5.) Non-Network Hospitals Level of Coverage: 60% of UCR charges. 6.)
