Center of Excellence Level of Coverage: 100% of contracted rates; $500.00 for family expenses for each day of in-patient medically necessary care (must be pre-approved by Executive Director) (Stanford Medical Center , Sequoia Hospital , O’Conner Hospital , University of California—San Francisco Medical Center and Children’s Hospital , and other pre-approved hospital facilities for specialized procedures and/or treatments) 3.) Primary Network Hospitals Level of Coverage: 90% of contracted rates. (Columbia Good Samaritan Hospital {San Jose}, Columbia San Jose Medical Center, El Camino Hospital/Camino Healthcare Systems {Mountain View}, Lucile Salter Packard Children’s Hospital at Stanford {Palo Alto}, San Jose Health Care Systems, Santa Clara Valley Medical Center {San Jose}, and St. Louise Regional Medical Center {Gilroy}. Salinas Valley Memorial Hospital is a Network facility effective April 1, 2011. 4.) Network Hospitals Level of Coverage: 80% of contracted rates. 5.) Non-Network Hospitals Level of Coverage: 60% of UCR charges. 6.) Regional Non-Network Hospitals Level of Coverage: 60% of UCR charges after an $2,500.00 admission fee (except for life-threatening emergencies) *Community Hospital of the Monterey Peninsula — CHOMP *Salinas Valley Memorial Hospital — SVMH
